Portington Cheif Carpenter vnto her matie & wm Ballett Goldsmith Churchewardens of ye p'ishe of St M'tine in the ffeildes in ye Countie of Midd', Chosen & appointed, wth ye whole Assent & Consent of ye Maisters of ye same p'ishe in vestrie for ye space of twoe whole Yeares Comensing at ye feaste of the beirth of or Lord god, 1595, & ending at ye like feaste in ye Yeare of or Lord, 1597, & in ye xlth yeare of ye Reign of or souereign Ladie Elizabeth by ye grace of god of England ffra'nc and Ireland Queene Defendor of ye faith &c' aswell for Receiptes as pay mentes by ym receaved & pd as in ye their accoumpt more p'ticularly ensueth.
