Confirmation of the foundation by Thomas de Sibethorp, rector of Bekayingham, in the diocese of Lincoln, of two chapels in the church of Sibethorp dedicated to St. Mary the Virgin and St. Ann for seven chaplains and two clerks, of whom one of the chaplains shall be called warden, to celebrate divine offices for the souls of the kings of England and all the faithful; together with statutes touching lights of wax and oil, and a dole of bread to be given every Monday, Wednesday, and Friday. The patronage of the church, value 20 marks, was transferred from the Hospitallers to the said warden and chaplains, and the whole was confirmed by William, archbishop of York.

The foundation charter is dated Southwark, 8 Id. Oct. 1331. The bishop's letters are dated Worcester, 30 July 1336; those of the archbishop are dated Leicester, 1 October 1336. Master John Geraudi was rector of the parish church, on the south side of which the chapel was built.
